An exciting opportunity has arisen for a highly skilled and dedicated Qualified Adult's Mental Health Social Worker to join a supportive, co-located Multi-Disciplinary Team (MDT) in the Blackburn with Darwen area. You will be responsible for providing expert social work services, promoting recovery, independence, and social inclusion for adults experiencing severe and enduring mental health conditions. This role requires a confident practitioner with a thorough understanding of relevant social care and mental health legislation.
Job Responsibility:
Undertake Statutory Assessments: Complete high-quality, person-centred social care assessments under the Care Act 2014, focusing on needs, strengths, and desired outcomes
Case Management: Manage a complex caseload, prioritising work based on need, risk, and promoting positive risk enablement
Safeguarding: Lead on adult safeguarding enquiries (Section 42), developing and implementing robust safeguarding plans in line with multi-agency procedures
Legal Compliance: Apply knowledge of the Mental Capacity Act 2005 (including undertaking Best Interest Assessments/BIAs if qualified), the Mental Health Act 1983 (including participating in AMHP duty if qualified), and relevant Human Rights legislation
Care Planning: Develop, implement, and review comprehensive care and support plans in collaboration with the service user, their families, carers, and the MDT
Multi-Agency Working: Work collaboratively within the MDT (including Psychiatrists, Nurses, Occupational Therapists, etc.) and with external agencies (e.g., housing, voluntary sector) to ensure a holistic approach to care
Advocacy & Rights: Champion the rights and choices of service users, advocating on their behalf to ensure they receive their entitlements and are treated with dignity and respect
Record Keeping: Maintain accurate, timely, and detailed records using electronic case management systems
Requirements:
A Degree/Diploma in Social Work or equivalent
Current and valid registration with Social Work England (SWE)
Significant post-qualifying experience (minimum of 1-2 years typically required for locum rates) in an Adults Mental Health or Adult Social Care setting
Confident and demonstrable understanding of the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983
Excellent communication, assessment, analysis, and report-writing skills
The ability to manage a challenging workload autonomously is crucial
Full UK driving licence and access to a car for travel across the Blackburn with Darwen area (essential for home visits and community work)
