Brook Street Social Care are seeking a dedicated and compassionate Project Worker to join our client's Supported Living Service in Lancaster on a temporary-to-permanent basis. This is a rewarding opportunity to support adults with complex mental health needs, helping them to achieve greater independence, stability, and improved wellbeing. The successful candidate will work closely with individuals experiencing a range of challenges, including mental health conditions, homelessness, substance misuse, housing instability, and social exclusion. As a Key Worker, you will build positive, professional relationships with residents, providing person-centred support and empowering individuals to work towards their personal goals.
Job Responsibility
Manage a caseload of residents and act as their designated Key Worker
Develop, implement, and review person-centred support plans and risk assessments
Support residents with housing applications
Assist individuals in accessing mental health services, healthcare, benefits, housing providers, and other community resources
Promote recovery, independence, and social inclusion
Work collaboratively with external agencies, including mental health teams, local authorities, housing providers, probation services, and substance misuse services
Maintain accurate records, case notes, and outcome-focused documentation
Safeguard vulnerable adults and contribute to effective risk management practices
Requirements
Experience working within supported housing, homelessness services, mental health services, substance misuse services, or a related field
Knowledge of recovery-focussed and trauma-informed approaches
Experience managing challenging situations professionally and calmly
Strong communication, organisation, and interpersonal skills
The ability to work independently and as part of a multidisciplinary team
A good understanding of safeguarding and professional boundaries
A commitment to equality, diversity, and inclusion
A relevant qualification in Health and Social Care, Mental Health, Housing, or a related discipline is desirable but not essential
