Job Description

You will play a key part in delivery of the preventative hubs in Llanelli, Amman and Gwendreath, these hubs provide preventative and solution-based community support to support customers to improve and/ or maintain their health, well-being and independence through social integration and strong community relationships.

Job Responsibility

  • Play a key part in delivery of the preventative hubs in Llanelli, Amman and Gwendreath
  • Deliver the single point of contact from which customers can access information, opportunities, and support
  • Work closely with your team and partner organisations
  • Receive referrals
  • Provide information
  • Have wellbeing conversations with customers
  • Deliver community engagement campaigns
  • Develop community support networks

Requirements

  • A passion to work with and enable people to live as independent a life as possible
  • A strong initiative and ability to work alone as well as being a great team player
  • Ability to deal with conflicting and difficult situations
  • Ability to build great rapport with customers
  • Ability to plan, deliver, review and monitor a holistic package of support
  • Ability to assess risk and propose risk management strategies
  • Hold a driving licence and have access to a car
