Job Description

Develops strategies, coordinates day to day operations, and evaluates impact of the services provided on patients referred to the program. Works with interprofessional team to facilitate overall programmatic growth.

Job Responsibility

  • Maintains the daily operations of the program
  • Coordinates care throughout the program utilizing appropriate tools to ensure high quality, cost effective care is provided to all patients referred to the program
  • Performs preliminary assessments and assigns service providers based on referral criteria
  • Acts as a resource person and liaises with others on matters related to the program and the program’s services
  • Partners with other clinical team members in assessing and supporting the needs of the patient/family who have been referred to the program
  • Presents program data and outcomes to internal and external audiences
  • Provides educational offerings to internal and external audiences on program services
  • Collaborates with management on the expansion of program offerings and services
  • Maintains program records and files including documentation in electronic medical record as appropriate

Requirements

  • Bachelor's Degree preferred
  • Registered Nurse (RN)
  • Three (3) years of experience in specific program area

  • Participates as a member of the Trauma Committee and prepares information for the trauma committee
