Job Description:

Join our team as a Primary Care Pharmacist and play a key role in improving the lives of patients with long-term conditions and minor ailments.

Job Responsibility:

perform comprehensive medication reviews, including structured medication reviews (SMRs)
independently prescribe treatments for minor ailments and long-term conditions
create and implement personalised care plans
collaborate with multidisciplinary teams to provide holistic care
maintain accurate and up-to-date patient records

Requirements:

a current and valid GPhC registration
Independent Prescriber (IP) qualification – essential
experience in primary care and treating minor ailments
strong communication and interpersonal skills
the ability to work independently and as part of a multidisciplinary team
a commitment to delivering high-quality patient care
