Registered with the GPhC as a Pharmacist
Independent Prescriber status – essential
Prior experience in a general practice or PCN setting
Strong clinical knowledge and patient communication skills
Ability to work both independently and as part of a wider healthcare team

Job Responsibility

Deliver medication reviews to enhance prescribing safety and efficiency
Support the management of chronic conditions such as hypertension, asthma, and diabetes
Prescribe autonomously as an Independent Prescriber
Collaborate closely with GPs, practice nurses, and other team members
Ensure accurate clinical documentation and record-keeping
