We are looking for an experinced Nurse with excellent communication and interpersonal skills and proven team working abilities, to be resourceful, with the ability to manage their own time effectively and prioritise their own workload. You will be part of a team that will work with care homes to enhance quality of care. Our focus is to enhance care in Care Homes by covering all aspects of care. You will be working within a wide skill mixed team, who are passionate about people living in a care home, taking both a medical and holistic approach. The post holder will be required to travel in between care homes and have a flexible approach to their day.
Job Responsibility:
Managing a case load of care home residents across a number of care homes
Provide care for the presenting patient, to include initial history taking, clinical assessment, diagnosis, treatment and evaluation of care
Demonstrate safe, clinical decision making including assessment and diagnosis skills
Carrying out regular planned (and in some cases) acute emergency visits
Producing and reviewing ACPs, TEPs (RESPECT) and promote Restore 2 training
Proactively planning patient care and assessing ongoing patient needs
Regular liaison with assigned GPs whenever appropriate
Proactively working with Care Home and Nursing Home staff and management to promote ongoing planned patients care
Discussing end of life care with patients and relatives
Coordinating referrals to other services such as, chiropodist, dietician and tissue viability
Working within their professional boundaries
Be involved in weekly care home rounds
Hold clinics for noncare home patients at the surgery and work collaboratively with Bethesda's nursing team, carrying out general practice nursing duties efficiently
Requirements:
Possess excellent communication skills able to liaise with GPs, Home management and staff and with relatives
Be able to work both independently and as part of a team
Be comfortable seeking advice from GP colleagues
Be able to respond to emergencies and where necessary liaise with GPs about acute admissions
Planning and implementing individual patient care within the context of an interdependent healthcare team
Achieving a balance between the demands of direct patient care within existing resources and job plan
Communicating with and supporting distressed/anxious/worried relatives /patients
Continually reappraise the effectiveness of resources against the unpredictability of service demands
Work within a specialty with unpredictable levels of activity
Providing a professional advisory role to a wide variety of contacts e.g. patients, relatives, carers, junior and senior nursing, medical and paramedical colleagues
To participate in reporting, documenting and evaluation of the work of the Care Home Clinician and to demonstrate its importance both to patients and members of the interdependent healthcare team
Keep professionally up to date with continuing professional development

Must hold a full drivers licence and access to own transport
