Job Description:

Are you a Primary Care Network Pharmacist looking for a rewarding locum role in Leicester? We have an exciting opportunity for you! Join our client’s team as a Primary Care Network Pharmacist and play a key role in improving the lives of patients with chronic conditions, with a strong focus on diabetes management.

Job Responsibility:

Perform comprehensive patient assessments
Create and implement personalised care plans
Deliver evidence-based treatments to optimise patient outcomes
Collaborate with multidisciplinary teams to provide holistic care
Maintain accurate patient records

Requirements:

A current and valid GPhC registration
Independent Prescriber qualification
Substantial experience in a Primary Care setting
Proven experience managing diabetes patients
Strong communication and interpersonal skills
The ability to work independently and within a multidisciplinary team
A commitment to delivering high-quality patient care
