Job Description:

Join our GP client’s team as a Prescribing Pharmacist and contribute to improving patient outcomes in primary care. This role includes medication reviews, prescribing support, triage assistance, and collaboration with healthcare professionals.

Job Responsibility:

Conduct structured medication reviews
Provide prescribing support and advice
Support patient triage processes as required
Collaborate with GPs and other healthcare professionals to optimise care
Maintain accurate clinical records in line with NHS standards

Requirements:

An active Independent Prescriber qualification
Proven experience in a GP surgery or primary care setting
Competence in medication reviews and triaging
Strong clinical and communication skills
EMIS experience (preferred)
