Job Description

Join our GP client’s team as a Prescribing Pharmacist and contribute to improving patient outcomes in primary care. This role includes medication reviews, prescribing support, triage assistance, and collaboration with healthcare professionals.

Job Responsibility

  • Conduct structured medication reviews
  • Provide prescribing support and advice
  • Support patient triage processes as required
  • Collaborate with GPs and other healthcare professionals to optimise care
  • Maintain accurate clinical records in line with NHS standards

Requirements

  • An active Independent Prescriber qualification
  • Proven experience in a GP surgery or primary care setting
  • Competence in medication reviews and triaging
  • Strong clinical and communication skills
  • EMIS experience (preferred)
