Job Description:

A GP surgery is seeking an experienced and compassionate Qualified Practice Nurse to deliver high-quality, evidence-based care in Primary Care. The role focuses on chronic disease management, preventative healthcare, and health promotion, working flexibly as part of a multidisciplinary team.

Job Responsibility:

  • Nurse-led management of chronic conditions
  • Preventative healthcare, immunisations and cervical cytology
  • Health promotion and supporting lifestyle change

Requirements:

  • Registered Nurse with 2+ years post-registration experience
  • Minimum 1 year General Practice experience
  • Recent community nursing experience
  • Competence in immunisations, cervical cytology, ear care and minor surgery
  • Experience managing chronic conditions and working to protocols
  • Strong communication, leadership, IT and audit skills
