Job Description

Join a friendly, community-focused GP practice to deliver high-quality nursing care. Manage patients with long-term conditions, support elderly and housebound patients, and lead services including infection control, chronic disease management, vaccinations, and health promotion.

Job Responsibility

Provide routine and acute nursing care based on evidence-based practice (NICE, NSF)
Manage chronic diseases, implement treatment plans, and run disease-specific clinics
Conduct well-woman/man clinics, vaccinations, travel medicine, and minor procedures
Support infection prevention and control initiatives, audits, and policy compliance
Maintain accurate clinical records and chronic disease registers
Provide patient education and guidance on medications
Mentor the nursing team, coordinate rotas, and participate in practice initiatives
Assist with safeguarding, mental health referrals, and care for vulnerable patients
