Job Description:

Join a friendly, community-focused GP practice to deliver high-quality nursing care. Manage patients with long-term conditions, support elderly and housebound patients, and lead services including infection control, chronic disease management, vaccinations, and health promotion.

Job Responsibility:

  • Provide routine and acute nursing care based on evidence-based practice (NICE, NSF)
  • Manage chronic diseases, implement treatment plans, and run disease-specific clinics
  • Conduct well-woman/man clinics, vaccinations, travel medicine, and minor procedures
  • Support infection prevention and control initiatives, audits, and policy compliance
  • Maintain accurate clinical records and chronic disease registers
  • Provide patient education and guidance on medications
  • Mentor the nursing team, coordinate rotas, and participate in practice initiatives
  • Assist with safeguarding, mental health referrals, and care for vulnerable patients
