Job Description

Service Care Solutions are recruiting for an experienced Practice Nurse to support a Primary Care Centre in Burgess Hill on a part-time basis. This is an excellent opportunity for a skilled clinician with strong experience in Chronic Disease Management and Wound Care to join a supportive primary care team.

Job Responsibility

  • Deliver routine nursing care and treatment to patients within agreed clinical protocols
  • Undertake chronic disease management clinics, including diabetes, asthma, COPD, hypertension, and cardiovascular disease reviews
  • Perform health assessments and NHS Health Checks
  • Administer routine vaccinations in accordance with national guidelines
  • Carry out cervical cytology screening
  • Provide wound care, dressings, and management of minor injuries
  • Undertake venepuncture, ECGs, spirometry, blood pressure monitoring, and other clinical procedures as required
  • Promote health education and support patients in managing long-term conditions effectively

Requirements

  • Registered Nurse with current NMC registration and experience working within a GP Surgery
  • Experience working within General Practice or Primary Care
  • Strong experience in Chronic Disease Management and Wound Care
  • Competent in immunisations, cytology, and wound dressing
  • Confident using EMIS
