Exciting Job Opportunity for Practice Nurse at Belmont Medical Centre, Hereford (WBC PCN). We are expanding our nursing team and seeking a Practice Nurse to join our forward-thinking, happy community-based GP surgery. Job Responsibility:
Deliver evidence-based nursing care to patients with acute and chronic conditions
Deliver clinics for long term condition management
Perform reviews in line with protocols and best practice
Provide immunisations, vaccinations, cervical cytology, wound care, travel health advice, and lifestyle support
Ensure accurate clinical documentation in line with NMC standards
Work with the wider MDT to ensure coordinated care and appropriate referrals
Participate in protocol development, audits, and service evaluation when required
Engage in professional development, mandatory training, and uphold the NMC Code
Requirements:
NMC Registration
Current UK professional registration
Adult Nursing Qualification -degree level
Understanding and knowledge of policy developments related to the delivery of primary care services including General Practice, the GMS contract, Clinical Governance, Quality & Outcomes Framework
Understanding of evidence based practice
Knowledge of national standards that inform practice (eg National Service Frameworks, NICE guidelines etc)
Understanding of their accountability arising from Codes of Professional Conduct and medico-legal aspects of the PN role
Understanding of equal opportunity and diversity issues
Experience in managing chronic conditions in a nurse-led capacity
Understanding of the role of a Practice Nurse in general practice
Experience implementing clinical protocols and guidelines
Experience with immunisations, wound care, cytology, and ABPI
Effective time management and ability to work independently
Confident in communicating sensitive or complex information to patients and families
Nice to have:
Experience of working in a GP practice using Emis
Experience of use of a medical software package
Ability to think strategically
Experience of presenting information to wider audience
Teaching / Mentoring experience and /or qualification
Independent Prescriber
