We’re looking for an innovative and experienced social work professional to join our Hospital Team as a Practice Lead. You’ll have strong leadership and supervisory skills, with experience in strengths-based practice, supporting Social Workers, Social Care Assistants, and Student Social Workers. The ideal candidate thrives in a fast-paced, pressured environment, brings fresh ideas to improve practice, and has a solid understanding of hospital discharge processes, the Discharge to Assess (D2A) pathway, and funding streams to ensure cost-effective care planning.
