At Oak Street, Population Health drives both our clinical and economic model and is focused on delivering high-quality, cost-effective care. The Population Health Director (PHD) will be instrumental in designing and implementing key Population Health priorities. The PHD will work on programs and interventions, targeted to the right patients, to improve quality and cost of care. The PHD will be an effective cross team leader, working side by side with providers, care teams, operations team members, and Treehouse team members. Content areas for the Director will include, but not be limited to, transitions-in-care, pharmacy and medication adherence, HEDIS / Stars, accurate clinical documentation, and medical cost management – with a focus on avoidable admissions. This will require building relationships with our clinicians, including primary care physicians, nurse practitioners, pharmacists, psychiatrists, registered nurses, licensed clinical social workers, and others. This is a fast-paced, high visibility role to ensure that we are driving innovation and improvement rapidly and consistently within an Oak Street division. This role will be aligned with a geographic division to rapidly drive results. This role will report directly to the Regional Vice President with a dotted line to the central Population Health team.
Job Responsibility:
Own strategic planning, execution and the ongoing refinement of Population Health programs in the respective region
Work side by side with regional leaders and center care teams to support the Oak Street care model including but not limited to clinical documentation excellence, workflow optimization, and avoidable admission prevention
Develop creative solutions to support accurate clinical documentation and avoidable admission prevention strategies that are scalable across multiple centers
Work closely with functional leaders in the central office to develop strategy and program design for accurate clinical documentation and medical cost / admission efforts
Analyze data to monitor performance, identify opportunities, and drive results toward population health execution in the region
Be accountable for medical scribe cohort recruitment, retention, performance and engagement in the respective region to ensure clinical documentation excellence
Be accountable to key population health metrics including but not limited to quality measures, utilization measures, and financial measures

Requirements:
5+ years of applicable experience in healthcare / population health / working with healthcare providers
or other similar fields
Excellent communication skills
Ability to build the trust with a wide range of stakeholders and hold teams accountable
Demonstrated history of strong leadership and data driven decision making
Incredible level of organization, initiative, and collaboration
Excellent analytical skills with ability to develop business cases, ROI analysis, and financial analysis
Expert in Excel, Google Apps, project management
Quick learner comfortable with a high level of ambiguity
Bachelor's degree required
US work authorization
