Job Description:

The Population Health/Wellness Case Management Coordinator utilizes critical thinking and professional judgment to support the case management process, in order to facilitate and maintain improved healthcare outcomes for members by providing advocacy, collaboration coordination, support and education for members through the use of care management tools and resources.

Job Responsibility:

Utilize critical thinking and professional judgment to support the case management process
Facilitate and maintain improved healthcare outcomes for members
Provide advocacy, collaboration coordination, support and education for members
Use care management tools and resources
Evaluate members through care management assessments and information/data review
Recommend approaches to resolving care needs
Identify high risk factors and service needs
Implement early and proactive support interventions
Coordinate and implement Wellness care plan activities
Monitor member care needs
Consult with case managers, supervisors, Medical Directors
Present cases at case conferences
Identify and escalate quality of care issues
Utilize negotiation skills to secure appropriate options and services
Utilize influencing/motivational interviewing skills
Provide coaching, information and support
Help members participate in healthcare decision-making
Utilize case management and quality management processes

Requirements:

Experience and detailed knowledge of the Foster Care and juvenile justice systems
Experience and detailed knowledge of Adoption Assistance
Experience and detailed knowledge of the delivery of Behavior Health Services
Experience and detailed knowledge of Trauma-informed Care
Experience and detailed knowledge of ACEs
Experience and detailed knowledge of Crisis Intervention services
Experience and detailed knowledge of evidence-based practices applicable to the Kentucky SKY populations
2 years' experience in behavioral health, social services or appropriate related field equivalent to program focus
Computer literacy and demonstrated proficiency
Ability to navigate through internal/external computer systems
Proficiency in MS Office Suite applications, including Word and Excel
Flexibility to work beyond core business hours of Monday-Friday, 8am-5pm
Valid KY driver's license
Proof of vehicle insurance
Reliable transportation
Minimum of a Bachelor's degree or a non-licensed master level clinician
Degree in behavioral health or a relevant human services field of study (social work, psychology, marriage and family therapy, counseling)
