Job Description:

You will join a dynamic Restorative Care Team to deliver high-quality Physiotherapy services in the Inner West Sydney Community. This includes providing client-centred, evidence-based care with a broad caseload. The role offers autonomy to structure work and support from an interdisciplinary team while ensuring care quality improvements consistent with HammondCare values and AHPRA standards.

Job Responsibility:

Provision of a high-quality Physiotherapy service to HammondCare's Support at Home clients including individual 1:1 home visits and group activities
assess and provide in-home rehabilitation programs to improve functional independence and quality of life
collaborate closely with our skilled IDT to assess clients’ functional care needs, set care goals, and provide treatment and interventions to deliver the best possible client outcomes
participate as a member of the HammondCare team in the application of the Mission and values to the pursuit of its objectives in a person-centred, continuous quality improvement work environment
participate in quality improvement projects and continuing professional development
provide clinical supervision for students, if appropriate

Requirements:

Tertiary qualified physiotherapist, with current AHPRA registration and practical experience or sound knowledge of physiotherapy in a Rehabilitation or Aged care setting
strong communicator, and able to plan and manage your time effectively
enjoy working independently and as part of a multi-disciplinary team
access to a reliable vehicle, a valid driver’s license and safe driving record to travel between client's homes and HammondCare locations
national police check
