An exciting opportunity has arisen for a Band 6 Physiotherapist to join Nottingham CityCares Integrated Respiratory and Home Oxygen Service to work 30 hours per week in a variety of settings. The Integrated Respiratory and Oxygen Service are a very proactive, forward thinking and dynamic team which provide evidence based assessment and follow up for individuals requiring urgent and routine respiratory management and provision of oxygen therapy in the community as well as rehabilitation and education in both home and clinic settings. We are looking for enthusiastic, motivated and conscientious physiotherapist with respiratory experience. The team would provide the successful applicant with the opportunity for postgraduate and specialist training to enable them to build on their current respiratory skills enabling them to support Respiratory patients to be independent in their community. Applications welcomed from part time applicants. The Service hours are 7 days per week 365 days per week between the hours of 8am and 7pm. Job Responsibility:
To assess respiratory patients and provide appropriate care including Pulmonary Rehabilitation sessions or home based treatment depending on patient agreed goals
To provide specialist advice, assessment and treatment for patients with a respiratory condition within the community and to support patients in respiratory exacerbation in an aim to avoid unnecessary hospital admissions
To provide specialist advice, assessment and treatments for patients being assessed for, or already on oxygen with respiratory and non-respiratory conditions
To work cohesively with other health professionals/agencies through the respiratory service referral process in order to ensure effective treatment management and planning for those with a respiratory condition
As an autonomous practitioner you will undertake comprehensive respiratory and/or home oxygen assessment for patients requiring urgent and chronic respiratory disease management or oxygen therapy
To undertake acute visits for patients needing assessment and advice
To educate and advise patients under the Integrated Respiratory and Oxygen Service patient pathway and those involved with working with the patient including other health and social care workers, students or carers
To be responsible for maintaining high standards of clinical practice utilising evidence-based practice
To provide line management responsibilities and mentorship to clinical staff and peers working within the specialty
To work collaboratively and across organisational boundaries to provide evidence-based practice and holistic care to patients in a variety of settings
To be responsible for the safe use of equipment and physical resources in the course of clinical duties
To work closely with community and secondary care providers to identify and manage patients with a respiratory condition at risk of frequent admission into hospital
To work closely with community and secondary care providers to identify and manage patients requiring Home Oxygen
To be based flexibly within the community whilst maintaining close links and regular communication with the hospital team
To provide specialist care and advice to patients with a respiratory condition undertaking full respiratory assessment, chest auscultation and interpretation of findings in order to manage urgent respiratory exacerbations and ongoing chronic respiratory management in the community/home setting
Utilise enhanced assessment skills and evidence based knowledge to provide specialist diagnostic service delivery in the clinic setting in relation to accurate diagnosis, interpretation of blood results, blood gases and the management of chronic respiratory conditions
To provide clinical, emotional and psychological support to patients and their families
To develop and co-ordinate individualised patient management plans, including a respiratory self-management plan, in partnership with the patients and their carers/family
To work as a member of an Integrated Respiratory and Home Oxygen Service Team in order to deliver a coordinated service to patients, in the community
To participate in Pulmonary Rehabilitation courses for patients, including patient education sessions held in a range of clinical settings
To act within the guidelines provided by the NMC Code of Professional Conduct
To contribute to the development and implementation of service protocols, standard operating procedures and service development projects
To provide information, support and referral to the smoking cessation service
To provide assessment, support and management of patients on Long Term Oxygen Therapy (LTOT) and Ambulatory Oxygen Therapy (AOT)
To participate in identified data collection and audit collaboratively with the team clinicians and the team manager in order to make recommendations for service change
To undertake comprehensive evidence-based risk assessments developing risk management plans
To establish and maintain effective channels of communication throughout the integrated team and partnership agencies
To liaise with Acute and Primary Care staff and integrated teams in order to ensure the patients holistic needs are met
Advise Lead responsible Clinician, of any problems occurring whilst the patient is being visited at home and act upon the decision as appropriate
To take responsibility for the appropriate delegation of work according to the skills and abilities of other team members
To be responsible for own Continuing Professional Development in an aim to keep abreast of new developments through continued training/conference attendance, whilst maintaining HCPC registration
To organise and participate in appropriate seminars and study days, acting as a specialist resource in Respiratory and Home Oxygen care
To contribute to the education and support of other clinicians within the primary care setting
To contribute to raising awareness of the role of the Integrated Respiratory and Oxygen Service, including the education and training for patients and other health care professionals
Ensure an approach to the care of Respiratory and Oxygen patients that minimizes risk and maximizes clinical effectiveness
Assist with the investigation and reporting of all incidents relating to the service, ensuring appropriate action is taken
Ensure an approach to care, which recognises the unique needs and views of Home Oxygen and Respiratory patients and their carers and which places the patient at the centre of all care delivery
To participate in and contribute towards any related research projects that have been formally agreed within limits of time and funding
Ensure that evidence-based findings are translated into practice
To collect and utilise data to enable monitoring and evaluation of the service, which will provide evidence of the value and effectiveness of the service
Supporting the use of clinical audit as a means of enhancing clinical practice and being receptive to the audit plans of others
To work collaboratively and flexibly within the Integrated Respiratory and Oxygen Service and provide essential cover in the absence of the other Integrated Respiratory and Oxygen Service Clinicians
To undertake supervision of peers and junior team members according to CityCare Supervision policy
To participate in any other agreed duties that may be reasonably required in addition to those above
Requirements:
Post graduate experience of caring for patients with a variety of different Respiratory conditions including Asthma, COPD, Bronchiectasis and Interstitial lung disease
Willingness to learn or post registration experience of carrying out a full respiratory patient assessment
Willing to undertake Ear Lobe Blood Gas Competency Package
Understanding of multi- disciplinary and/or multi agency working within a community setting
Experience of working between primary and secondary care setting
BSc Hons Physiotherapy or equivalent
HCPC registered Physiotherapist
Able to work effectively in a team, supporting others and challenging colleagues views and attitudes when necessary
Able assess a situation, set priorities and problem solve quickly and effectively
Awareness of HCPC professional codes of conduct
Excellent communication and interpersonal skills
Able to assess clinical risk
Effective time management skills
Evidence of development of clinical skills
