The program provides intensive, multidisciplinary rehabilitation in the home and community setting immediately following discharge. The Rehab at Home Senior Physiotherapist plays a pivotal role in managing and delivering physiotherapy services for patients receiving home-based rehabilitation care. Combining clinical expertise with leadership, this role focuses on improving patient outcomes while driving team development. As a senior member of the team, the position involves effective collaboration with key stakeholders during discharge planning, ensuring that realistic and achievable goals are set for patients' rehabilitation journeys.
