Job Description

Cpl UK Healthcare are currently recruiting for a Band 6 Physiotherapist to join a service in the Wirral working as a Falls Practitioner within a community-based setting. This is a part-time role (30 hours per week). The role involves working with patients at risk of falls, providing assessment, rehabilitation, and prevention strategies within community settings. You will support patients to maintain independence, reduce risk, and improve overall mobility and safety.

Job Responsibility

  • Manage a caseload of patients at risk of falls within the community
  • Complete comprehensive falls risk, mobility, and balance assessments
  • Develop and implement individualised rehabilitation and prevention plans
  • Carry out home and environmental safety assessments
  • Work collaboratively with the multidisciplinary team and community services
  • Provide education and advice to patients, families, and carers
  • Maintain accurate clinical records and outcome measures

Requirements

  • HCPC registration
  • Experience in physiotherapy, ideally within community, elderly care, or rehabilitation
  • Strong understanding of falls prevention, mobility, and balance
  • Ability to work independently and manage a caseload
  • Excellent communication and organisational skills
