An exciting opportunity currently exists for an experienced Grade 3 Physiotherapist to join the Emergency Department (ED) Care Coordination team at Western Health on a fixed-term, part-time basis working across Western Health’s multiple EDs on 7-day rotating roster (including both AM and PM shifts).
Job Responsibility:
Deliver Emergency Department Care Coordination within a transdisciplinary model of care
support timely, patient-centred assessment, intervention, and discharge planning for patients presenting to the Emergency Department
focus on reducing re-presentation
provide targeted physiotherapy-based assessments and interventions to patients in our short stay unit where clinically indicated
play a pivotal role in both direct clinical care and the professional supervision of your discipline
contribute to the strategic direction of the EDCC service
provide support to the Rapid Allied Health team by providing discipline specific assessment and intervention in patients’ homes to support safe discharge
Requirements:
Advanced clinical skills and minimum 5 years’ experience in the specified Allied Health discipline
current registration with AHPRA
ability to independently develop and deliver comprehensive evidence-informed practices at an individual and team level across diverse patient groups
demonstrated strengths in positively leading others and a solutions-focused mindset
experience initiating and managing quality improvement and/or research aligned with Western Health’s strategic plan
exceptional prioritisation, time management and problem-solving skills
strong interpersonal, written, and electronic communication abilities
commitment to ongoing professional development and lifelong learning
a current Victorian driver’s licence
willingness to undergo a police check
