Physician

• Provide In-Home Health Evaluations
• Engage with people in their homes
• Help bridge gaps in care
• Conduct medication and medical history review
• Perform physical evaluation
• Conduct diagnostic tests (diabetic eye exams, spirometry, etc.)
• Use iPad and clinical workflow for evaluations
• Travel locally and within licensed states
• Conduct virtual visits as needed

• Active, unrestricted license(s) in coverage area(s)
• Multi-state licenses (preferred)
• Able to cover the following primary locations: Monterey, San Benito, Santa Clara, and Santa Cruz counties
