Job Description:

Signify Health, part of CVS Health®, is seeking a Per Diem Physician to provide In-Home Health Evaluations. The role involves engaging with patients in the comfort of their own homes, reviewing medication and medical history, conducting physical evaluations, and carrying out diagnostic tests if ordered. Flexible coverage within licensed states and virtual visits are required.

Job Responsibility:

Conduct virtual visits as needed
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products
Work with Signify Health as a clinician colleague to fulfill requirements

Requirements:

Active, unrestricted license(s) in coverage area(s)
Multistate licenses (preferred)
Able to cover the following primary locations: Hartford, CT
