Orleans Community Health is seeking a highly motivated and compassionate Primary Care Provider to join our team in Medina, NY. Job Responsibility:

  • Provides high-quality primary care services to patients of all ages, including but not limited to diagnosis, treatment, and management of acute and chronic medical conditions
  • Performs routine health check-ups, physical exams, and health screenings
  • Develops and implements individualized treatment plans, including prescription medications and referrals to specialists when necessary
  • Educates patients and their families about health issues, preventive care, and healthy lifestyle choices
  • Maintains accurate and up-to-date medical records, in accordance with HIPAA regulations and best practices
  • Collaborates with other healthcare professionals to ensure the highest quality of patient care
  • Actively participates in community events and initiatives to promote health and wellness, and to build relationships with patients and community members

Requirements:

  • Degree in Medicine, Nursing, or Physician Assistant Studies from an accredited institution
  • Active and unrestricted license to practice in New York
  • 3-5 years of experience in primary care preferred, new grads welcome
  • Strong clinical skills and knowledge of current medical practices
  • Excellent communication skills and ability to build rapport with patients and their families
  • Commitment to providing compassionate and patient-centered care
  • Ability to work independently and as part of a team in a fast-paced environment
  • Demonstrated commitment to community involvement and engagement
