Job Responsibility

Perform skilled evaluations to assess functional abilities related to activities of daily living, work, and community participation
Develop and implement individualized treatment plans focused on improving physical, cognitive, and psychosocial function
Provide interventions for a variety of conditions including post-surgical rehabilitation, neurological disorders, and chronic health conditions
Collaborate with physicians, nurses, social workers, and other allied health professionals to deliver integrated care
Educate patients, families, and caregivers on home exercise programs, adaptive equipment, and strategies for independence
Maintain accurate, timely, and culturally respectful documentation in compliance with organizational and regulatory standards
Participate in community health initiatives, prevention programs, and wellness education as needed
