Job Description

Under the general guidance from the Executive Director of APCs with medical oversight from the MIH Physician leadership, the Physician Assistant or Nurse Practitioner will provide telehealth care to patients in the community. Rapidly growing service line adding provider coverage to meet the growing need in the community. the NP/PA will evaluate, diagnose and treat patients following written guidelines approved by the Department of Medicine.

Job Responsibility

  • Provide telehealth services to patients and community partners to operationalize COVID testing and/or discharge follow ups after ED/Hospitalization for COVID symptoms
  • Telehealth evaluations and coordination for community based testing and incoming phone requests from SNFs, ALFs and Psychiatric Hospitals
  • Provider will complete Provider to Provider screening and place orders for testing for symptomatic home-bound and at-risk populations
  • Documentation in SSH MIH Epic Code for provider to provider consultation Provider will coordinate with Registration to assure patient information is updated in Epic Provider will coordinate with EMS Dispatch to print lab labels and dispatch MIH/EMS to collect specimens
  • Telehealth follow up with discharged patients
  • Documentation in SSH MIH Epic Code for Telehealth visits (telephone or video)
  • Provider will determine next course of action based on Telehealth visit
  • Telehealth and consultation with MIH paramedics
  • Participates in continued learning and possess a willingness and ability to learn and utilize new technology and procedures that continue to develop in their role and throughout the organization
  • Embraces technological advances that allow us to communicate information effectively and efficiently based on role

Requirements

  • License/Registration/Certification Requirements: ACLS (AHA) Advanced Cardiac Life Support Certification - American Heart Association (AHA) (Including courses offered through SSH)
  • Certified Nurse Practitioner - Board of Registration in Nursing (Massachusetts)
