Meets the discipline-specific productivity standard
Demonstrates competency in the care of patients during field observation by the clinical supervisor
Completes and submits documentation following established guidelines
Ensures physician orders are present and written correctly
Documents follow up of identified problems
Reviews and documents clinical progress toward goals
Demonstrates coordination of care by appropriate communication with other disciplines and team members, and documents that communication
Meets agency/regulatory requirements
Documents appropriate management of visit utilization and authorization status
Meets regulatory requirements for supervisory visits
Requirements:
Completed an accredited physical Therapy degree program and be board eligible and able to obtain CA PT license within 6 months or have already obtained a CA PT License
Previous experience as a Physical Therapist or Physical Therapist student/intern, upon hire
Physical Therapist: CA, within 6 - months
Basic Life Support - CPR, upon hire
Driver's License: CA, upon hire
