Job Description:

We are looking for a licensed Physical Therapist to join our compassionate and growing team in Fond Du Lac. This is a great opportunity to build meaningful connections with patients while enjoying autonomy and flexibility in your schedule. As a clinician with PRN HH&T, you’ll primarily work in a SNF setting, with additional opportunities for cross-training and work in Home Health if desired.

Job Responsibility:

Perform patient evaluations in a SNF setting and develop individualized treatment plans
Deliver evidence-based therapy to improve patient mobility, function, and quality of life
Collaborate with patients, families, and clinical team members to support care goals
Accurately document treatments and progress, including discharge summaries
Supervise and guide Physical Therapist Assistants (PTAs) in treatment plans
Educate patients and caregivers on safe mobility practices and discharge planning

Requirements:

Current Physical Therapist (PT) license in the state of Wisconsin
Valid driver’s license and reliable transportation
SNF or Home Health 1-3 years of experience preferred but not required—we provide training!
