Under the supervision of the Director of Rehabilitation Services, the physical therapist assumes responsibility and accountability for the process of evaluation, assessment, diagnosis, planning and implementation needed to deliver safe, evidence based care. Position provides patient care for the Home Health department to promote excellence in patient care.
Job Responsibility:
Evaluates patients in clinical area served
Develops and implements treatment plans
Documentation in compliance with departmental, KPTA and APTA guidelines, and regulatory requirements
Demonstrates understanding of regulatory requirements
Meets all state requirements for continuing education
Demonstrates good rapport with patients and professionalism
Provides supervision for PTA’s and rehabilitation technicians
May provide educational programs through in-services
May provide student supervision
May assist to develop, collect, and analyze performance improvement data
Assumes responsibility for risk and safety issues
Utilizes resources efficiently and effectively
Takes responsibility for own learning needs
Participates as a member of a team to achieve organizational goals
Requirements:
Bachelors Degree in Physical Therapy OR
Master's Degree in Physical Therapy OR
Doctorate in Physical Therapy
Licensed Physical Therapist (PT) - State Board of Healing Arts
Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)
