Job Description:

Orleans Community Health at Medina Hospital is looking for an experienced Physical Therapist to join our team in Medina, New York. If you are a caring individual who enjoys working in a fast-paced rehab setting then we’d love to talk with you!

Job Responsibility:

  • Evaluates and re-evaluates patient’s musculoskeletal, neuro, and functional levels based on patient’s presenting condition and diagnosis
  • Plans a written treatment program and treatment goals based on patient evaluation/re-evaluation
  • Administers treatment plan (group or individual) utilizing modalities, manual skills, therapeutic exercise and/or functional activities/training
  • Completes documentation and routine paperwork on appropriate forms within prescribed time frames
  • Educates patients, families, caregivers, peers, students, and community members according to specific needs
  • Participates in team communication in regards to patient care issues and hospital/departmental functions
  • Maintains surveillance of environmental conditions as related to safety and infection control
  • Completes other and/or additional job related tasks as requested

Requirements:

  • Graduate of an accredited school approved by the American Physical Therapy Association with a BS. MS or Doctorate degree in Physical Therapy
  • Registered and licensed by the State of New York in Physical Therapy with a valid permit to practice Physical Therapy in New York State
