Job Responsibility:

  • Evaluates patients, selects and administers assessment techniques, synthesizes information to establish appropriate patient care plan to include necessary modalities, procedures, exercise programs, therapeutic groups and patient education as warranted by a holistic view of the patient's diagnosis, condition, age, and discharge needs
  • Gathers and interprets appropriate data from the medical record, physician reports, patient, and/or family as evidenced by documentation
  • Documents evaluations, tests, progress notes, daily treatment notes, attendance records, patient/family education, outcomes, discharge notes, and charges as needed
  • Communicates patient's evaluation and subsequent progress to physician, referral sources, and other clinical staff, coordinates patient's schedule and treatment programs, and discharge planning
  • Provides and documents patient and family education in regard to deficits, progress toward goals, and therapeutic techniques, with individualized instruction for a home program including equipment use
  • Directs and supervises activities of Physical Therapist Assistant, Physical Therapy Technician, students, and volunteers and assists with orienting new and rotating staff

Requirements:

  • Doctorate degree in Physical Therapy from an accredited school or educational requirements commensurate with the date of graduation
  • Maintains continuing education requirements as mandated by applicable state professional standards
  • Licensed Physical Therapist in the State upon which you work
  • Basic Life Support (BLS) for Healthcare Providers (obtained within 90 days unless department leader has determined is not required) by American Heart Association (AHA)
  • No experience required
  • Ability to operate expertly the ultrasound equipment with specialized Cardiology software and a desktop computer is required
  • Advanced knowledge of cross-sectional anatomy, physiology and pathology as related to cardiac circulation required

  • Includes documentation of supervisory activities in the patient's medical record
  • Monitors and documents the patient's response to physical therapy intervention
  • Participates in patient care conferences, in services, chart audit activities, quality improvement activities including OBQI and OBQM and peer-review activities
  • Maintains confidentiality of information relating to the physical therapist-client relationship
  • Makes recommendations for follow-up care in coordination with the agency that admitted the patient and utilizes other agency care providers to assist with referrals to community resources
  • Performs other duties as assigned
