Job Responsibility

  • Conduct comprehensive physical and/or occupational therapy assessments
  • Develop and implement individualized treatment plans based on client goals
  • Provide therapeutic exercises, functional training, and rehabilitation techniques
  • Assist clients in improving mobility, strength, coordination, and daily living skills
  • Train clients in the use of adaptive equipment and assistive devices
  • Educate clients and caregivers on therapy plans, exercises, and safety practices
  • Monitor and document client progress and modify treatment plans as needed
  • Collaborate with physicians, nurses, caregivers, and other healthcare professionals
  • Maintain accurate and timely clinical documentation in compliance with regulations
  • Promote client independence and safety in home or care settings
  • Participate in care planning meetings and case conferences
  • Participates in client teaching according to the client’s plan of care
  • Assist in providing a safe environment and promote quality client care through adherence to established policies, procedures and standards
  • Ensure client’s rights are adhered to
  • Respond rapidly and appropriately in an emergency situation
  • Use equipment properly and in a safe manner
  • Perform other tasks as determined and instructed by the licensed professional and as permissible under Arizona law

Requirements

  • Degree in Physical Therapy or Occupational Therapy from an accredited program
  • Current state licensure as a Physical Therapist and/or Occupational Therapist
  • Previous clinical or home care experience preferred
  • Strong understanding of rehabilitation principles and practices
  • CPR/First Aid certification (as required)
  • Holds a valid level one Fingerprint card
  • Ability to travel daily including weekends
  • Understands regulatory compliance
  • Must be able to legally work in Arizona
  • Valid driver’s license is required
  • Treats clients, staff and the public with courtesy, respect and presents a positive public image
  • Works as a team member
  • Ensures confidentiality and security of the client’s medical information
  • Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull
  • Must be able to lift up to 50 pounds
  • Visual and hearing acuity required
