Job Description:

We are seeking a compassionate and independent Physical Therapist – Home Care to provide skilled therapy services to patients in their homes. The PT will evaluate, develop, and implement individualized treatment plans to improve mobility, restore function, reduce pain, and enhance overall quality of life in a home health setting.

Job Responsibility:

Conduct comprehensive patient evaluations in home settings
Develop and implement personalized home care treatment plans
Provide therapeutic exercises, gait training, balance training, and mobility support
Educate patients and caregivers on home exercise programs and safety techniques
Monitor patient progress and modify treatment plans as needed
Complete accurate and timely documentation in EMR systems
Coordinate care with physicians, nurses, occupational therapists, and case managers
Ensure compliance with home health regulations and agency policies
Participate in case conferences and discharge planning
Travel within assigned territory to provide patient care

Requirements:

Doctor of Physical Therapy (DPT) or equivalent degree
Active New Jersey Physical Therapist license
Minimum 1 year of PT experience (Home Health preferred)
Strong clinical assessment and time management skills
Valid driver’s license and reliable transportation

Nice to have:

Home Care / Home Health experience
BLS certification
Knowledge of OASIS documentation
Experience with geriatric and post-acute patients

Requirements

Current license to practice physical therapy specific to the state you're assigned to work in
Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE)
One year related physical therapy experience for PTs entering home health care
If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program
Current CPR certification

Provides interventions, conducts reexaminations (every 30 days, at minimum), modifies interventions as necessary, and performs discharge planning
