1 year hospital or retail pharmacy experience preferred (completion of an Accredited Technician training course may substitute for experience)
Working knowledge of computer systems
High school diploma or equivalent
Active Pharmacy Technician License (Kansas), Certification (Colorado or Utah), Provisional Certification (Colorado), or Provisional License (Utah) Required. Provisional Certifications must obtain State Certification within 12 months
Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly

CPHT - Certified Pharmacy Tech - PTCB - Pharmacy Technician Certification Board (required)
TCH-Pharmacy Technician License - CA State Board of Pharmacy (required)
Minimum three (3) years Hospital pharmacy, medical clinic, or retail pharmacy experience. (required)
Computer Skills Proficiency with the Microsoft Office Suite.

Job Responsibility

Participating in medication reconciliation for newly admitted patients identified as high risk for medication misinformation.
Obtaining medication history from the patient/caregiver and appropriate pharmacies.
Entering or updating the medication list in the electronic health record.
Communicating completion of the medication reconciliation and identified discrepancies to the patient's care team (physician, nurse and pharmacist).
Participating in Quality Assurance/Quality Improvement projects related to medication reconciliation.
