Job Description:

A unique opportunity awaits a seasoned pharmacist to join an outpatient clinic in a picturesque location while it transitions its EMR system from Meditech to Epic. Enjoy a dynamic work environment with a rotating schedule, covering both day and evening shifts, Monday through Friday, and alternate weekends. This role is pivotal in ensuring safe and effective medication management for a varied patient population.

Job Responsibility:

  • Prepare and dispense medication orders per physician request while managing medication stock
  • Ensure safe, effective medication therapies as per established policies and protocols
  • Interpret medication orders and maintain computerized patient medication profiles accurately
  • Compound and dispense a wide range of pharmaceuticals, including IV admixtures and chemotherapy
  • Monitor drug therapy regimens for contraindications and interactions, providing clinical consultations as needed
  • Participate in quality improvement and medication review activities, collecting data and maintaining logs

Requirements:

  • Active CO PHA License in good standing
  • Graduate of an ACPE-accredited School of Pharmacy with a BS Pharmacy degree
  • EPIC experience required
  • BLS and ACLS certifications from the American Heart Association
  • Proof of flu vaccination (required from Oct 1st through April 1)
  • Proficient in interpreting and transcribing complex medication orders
  • Capable of providing clinical consultation and clarification to practitioners
  • Skilled in preparing comprehensive clinical documentation and performing quality inspections
  • Experience with pharmokinetic drug dosing and monitoring drug efficacy and safety
  • Member of the Code Response Team and able to provide emergency support as needed
