Job Description:

Are you a Pharmacist (IP) looking for a rewarding locum role in South Wales? We have an exciting opportunity with great pay and flexibility!

Job Responsibility:

perform comprehensive medication reviews
manage hypertension and chronic disease screening
create and implement personalised care plans
collaborate with multidisciplinary teams to provide holistic care
maintain accurate patient records using EMIS and SystmOne

Requirements:

a current and valid GPhC registration
Independent Prescriber qualification
experience in medicines reviews, hypertension, and patient screening
strong communication and interpersonal skills
the ability to work independently and within a team
experience using EMIS and SystmOne
