Pharmacist

• Conduct Structured Medication Reviews (SMRs) and follow-ups
• Support long-term condition management (e.g., hypertension, asthma, diabetes)
• Manage repeat prescriptions, medicines reconciliation, and deprescribing
• Provide medicines-related advice to clinicians and patients
• Implement medicines optimisation initiatives and cost-saving audits
• Assist with QOF and IIF targets, improving prescribing safety and outcomes
• Use EMIS or SystmOne to maintain accurate patient records and documentation

• GPhC registration as a Pharmacist
• Minimum 6–12 months’ experience in a PCN or GP setting
• Strong working knowledge of EMIS or SystmOne
• Excellent communication, clinical judgment, and patient counselling skills
• Independent Prescriber status preferred but not essential
• Commitment to full-time availability (Monday to Friday, 9am–5pm)
