Job Description:

Hunter AHP has an excellent opportunity for a Clinical Pharmacist with PCN experience to join a forward-thinking GP surgery in Hampshire. This is a long-term full-time locum post ideal for a pharmacist looking to make a clinical impact within a supportive primary care team. The role includes managing clinical workload, contributing to service development, and delivering patient-facing clinics.

Job Responsibility:

Conduct Structured Medication Reviews (SMRs) and follow-ups
Support long-term condition management (e.g., hypertension, asthma, diabetes)
Manage repeat prescriptions, medicines reconciliation, and deprescribing
Provide medicines-related advice to clinicians and patients
Implement medicines optimisation initiatives and cost-saving audits
Assist with QOF and IIF targets, improving prescribing safety and outcomes
Use EMIS or SystmOne to maintain accurate patient records and documentation

Requirements:

GPhC registration as a Pharmacist
Minimum 6–12 months’ experience in a PCN or GP setting
Strong working knowledge of EMIS or SystmOne
Excellent communication, clinical judgment, and patient counselling skills
Independent Prescriber status preferred but not essential
Commitment to full-time availability (Monday to Friday, 9am–5pm)
