Pharmacist

• Conducting medication reviews for patients, including chronic disease management and polypharmacy cases
• Supporting repeat prescribing processes and ensuring safe, clinically appropriate prescriptions
• Collaborating closely with GPs, nurses, and other practice staff to improve patient care
• Providing medicines-related advice to the clinical team and patients
• Supporting audits, quality improvement initiatives, and clinical governance activities

• Experience as a Clinical Pharmacist in a primary care or GP surgery setting
• Ability to work independently and as part of a multidisciplinary team
• Strong communication and organisational skills
• Confident using EMIS or other clinical systems
