Job Description:

Join our client’s team as a Band 6/7 Pharmacist supporting an inpatient NHS hospital service. The role involves performing clinical checks and reviews on inpatient prescriptions, managing medicines reconciliation and discharge planning, and supporting ward-based clinical pharmacy duties.

Job Responsibility:

Perform clinical checks and reviews on inpatient prescriptions
Work alongside a multidisciplinary team to ensure safe, effective patient care
Manage medicines reconciliation and discharge planning
Support ward-based clinical pharmacy duties
Maintain clear and accurate records

Requirements:

GPhC registration as a Pharmacist
Strong communication and teamwork skills
A proactive and adaptable approach to patient care
Prior NHS hospital experience is desirable but not essential
