As our Pharmacist Resident, you will actively participate in an intensive post-graduate training program designed to develop significant knowledge and clinical skills for effective pharmaceutical care.Every day, you will engage in a variety of activities across clinical, operational, and administrative domains to apply your education to patient care situations. Job Responsibility
Participate in all aspects of comprehensive clinical pharmacy services including rounds with medical teams, pharmacokinetic consultations, parenteral nutrition consultations, anticoagulation management, antimicrobial stewardship monitoring, medication reconciliation, discharge counseling, documentation of clinical interventions in the identified data base, and compliance with clinical pathways and protocols
Performs all activities associated with meeting the goals and objectives of the American Society of Health-System Pharmacists (ASHP) accredited PGY1 residency program
Process prescriber orders in the identified pharmacy information systems in an effective, efficient, timely, safe and accurate manner to ensure the appropriateness of the indication, dose/frequency, route, interactions, dose adjustment, physical and therapeutic incompatibilities, side effects, duplicate therapies, and cost-effectiveness. Evaluate need for all non-formulary medication required
Provide direct oversight and timely and accurate verification of final product for all aseptic parenteral and non-parenteral medication preparation, dispensing and distribution in compliance with state, federal statutes and hospital procedures
Ensure doses are filled and delivered within established departmental guidelines in a timely and safe manner
Identify and evaluate adverse drug reactions and medication errors
Requirements
Graduation from an Accreditation Council for Pharmacy Education (ACPE) approved school of pharmacy including didactic courses and experiential rotations in the clinical aspects of pharmaceutical care, upon hire
Pharmacist Intern: TX, upon hire
Pharmacist: TX, within 90 Days
Basic Life Support - CPR, upon hire
