As our Resident Pharmacist, you will gain hands-on experience supporting patient care and medication management under supervision. You will apply your pharmaceutical knowledge to clinical and operational tasks, contributing to safe patient outcomes. Every day you will assist with dispensing, compounding, and medication reconciliation, learning workflows and policies. You will engage in patient counseling, drug information, and team collaboration, supporting pharmacist-led activities. To be successful in this role, you will be a recent PharmD graduate or pre-licensure, demonstrating strong knowledge, a proactive attitude, and excellent communication. Your dedication to growth, detail, and teamwork will be key.
Job Responsibility:
Participate in all aspects of comprehensive clinical pharmacy services including rounds with medical teams, pharmacokinetic consultations, parenteral nutrition consultations, anticoagulation management, antimicrobial stewardship monitoring, medication reconciliation, discharge counseling, documentation of clinical interventions in the identified data base, and compliance with clinical pathways and protocols
Process prescriber orders in the identified pharmacy information systems in an effective, efficient, timely, safe and accurate manner to ensure the appropriateness of the indication, dose/frequency, route, interactions, dose adjustment, physical and therapeutic incompatibilities, side effects, duplicate therapies, and cost-effectiveness
Evaluate need for all non-formulary medication required
Provide direct oversight and timely and accurate verification of final product for all aseptic parenteral and non-parenteral medication preparation, dispensing and distribution in compliance with state, federal statutes and hospital procedures
Ensure doses are filled and delivered within established departmental guidelines in a timely and safe manner
Identify and evaluate adverse drug reactions and medication errors
