Job Responsibility

  • Provides one-on-one phone/video based consultations with commercial and Medicare Part D plan members
  • Assisting with the intake/enrollment process
  • Consult with the patient to identify, resolve and prevent medication related problems
  • Assess each patient’s medications for indication, effectiveness, safety and adherence issues
  • Incorporate lab data in the assessment process when available
  • Prepare a medication summary and personal care plan with recommendations for the patient to share with their medical providers
  • Follow up with the patient to monitor drug therapy and ensure positive outcomes

Requirements

  • Arizona Pharmacist License in good standing
  • Must reside in the State of Arizona
