Job Description:

POSITION SUMMARY/RESPONSIBILITIES: Collaborates with pharmacy leadership, physicians and interdisciplinary team members in accordance with TJC, OSHA, Medicare/Medicaid standards and Texas State Board of Pharmacy Rules and Regulations to deliver pharmaceutical care to eligible University Health System patients in the ambulatory setting. Conducts D.U.E. and patient/physician education activities as necessary. Functions as a source of drug information. Serves as a primary preceptor for pharmacy students and residents. Implements decisions set forth by the P&T Committee as written in the Hospital Formulary and complies with Health System policies and procedures.

Job Responsibility:

  • Conducts D.U.E. and patient/physician education activities
  • Functions as a source of drug information
  • Serves as a primary preceptor for pharmacy students and residents
  • Implements decisions set forth by the P&T Committee
  • Complies with Health System policies and procedures

Requirements:

  • PharmD or Bachelor's of Science degree in Pharmacy required
  • Completion of an American Society of Health System Pharmacy PGY 1 residency or 3 years of applicable experience required
  • Must be licensed to practice pharmacy in the state of Texas
  • Must maintain an active preceptor license in the State of Texas

Nice to have:

  • Completion of an American Society of Health System Pharmacy PGY2 residency in ambulatory care preferred
  • Certification by the Board of Pharmaceutical Specialties in Pharmacotherapy or Ambulatory Care preferred
