Job Responsibility

Review and verify medication orders for accuracy, appropriateness, and potential drug interactions
Prepare and dispense chemotherapy drugs and other medications used in cancer treatment
Calculate drug dosages based on patient-specific factors such as weight, height, and organ function
Collaborate with healthcare professionals to develop individualized treatment plans and adjust medications as needed
Participate on an oncology treatment plan committee to aid in upkeep of EPIC® Beacon treatment plans and creation of new treatment plans
Assess and develop selection of evidence-based and therapeutically appropriate anticancer therapies based on diagnosis, performance status, organ function, disease staging and pertinent pharmacogenomic status
Monitor patient responses to therapy, assess adverse effects, and provide appropriate interventions
Work closely with oncologists, nurses, and other healthcare team members to optimize patient care and outcomes
Review primary literature resources and appropriate guidelines (e.g., NCCN, ASCO) when evaluating, recommending or implementing anticancer therapy and supportive care
Prospectively identify and triage chemotherapy and supportive medication issues such as dose adjustments, prior authorizations, etc.

Job Responsibility

Approves the ordering of medications for patients treated at the medical center
Oversees and directly contributes to compounding, preparation and dispensing of medications for patients in accordance with all applicable state and federal laws and regulations for Pharmacy practice

Understands and follows departmental and hospital policy and procedures related to dispensing of medications
