The Personal Support Worker (PSW) is a member of the interdisciplinary healthcare team responsible for providing direct personal care and support in accordance with patient care plans and the direction of the Patient Care Manager or supervising RPN. As part of Ontario Health’s Hospital to Home (H2H) program, the PSW supports patients transitioning from hospital to home, including Emergency Department patients and inpatients designated as Alternate Level of Care (ALC) or at risk of becoming ALC, who require restorative, reactivation, rehabilitation, or optimization care services. Working collaboratively with other healthcare disciplines, the PSW promotes safe and timely discharges, continuity of care, quality of life, independence, emotional well-being, and participation of family and community while incorporating rehabilitative principles and patient-centered care approaches.
Job Responsibility
Assists patients with activities of daily living (ADLs), including hygiene, toileting, feeding, mobility, exercise, and leisure activities
Supports safe hospital-to-home transitions, discharge readiness, and settling-in care
Assists with mobility, transfers, positioning, and restorative/reactivation activities to promote recovery and independence
Provides light meal preparation, feeding assistance, and light housekeeping to maintain a safe environment
Assists with colostomy, ileostomy, and ureterostomy care as directed
Monitors patient condition, takes vital signs, collects specimens, and reports changes promptly to the RPN or Patient Care Manager
Provides emotional support, companionship, and encourages social interaction and cognitive stimulation
Reinforces rehabilitation and restorative care plans developed by the interdisciplinary team
Maintains accurate documentation, confidentiality, infection prevention, and safety standards
Participates in interdisciplinary meetings, injury prevention, risk assessments, and quality patient care activities
Collaborates effectively with healthcare team members, residents, and families to support patient well being
Participates in ongoing education, performance improvement, and other assigned duties as required
Requirements
Personal Support Worker certificate from an accredited program
CPR and First Aid certification
Vulnerable Sector Check
Must have valid license and provide a good driving abstract
Must have own vehicle and valid insurance
Must be willing to provide in-home care
Excellent written and oral communication and interpersonal skills
Excellent organizational skills
Computer literacy required e.g., Microsoft Office
Nice to have
Experience in home care, rehabilitation, or transitional care is an asset
Experience in community nursing, transitional care, or rehabilitation preferred
