Job Description:

This role offers the opportunity to work in both our Memory Care and Traditional neighborhoods. Employees are fully cross-trained to provide consistent, resident-centered care throughout the community.

Job Responsibility:

  • Assist residents with activities of daily living (ADLs) including bathing, dressing, grooming, toileting, and mobility
  • Assist residents with self-administration of medications, including proper documentation
  • Observe and report changes in residents’ physical or emotional condition to nursing staff
  • Provide supportive, respectful care that meets residents’ physical, emotional, and psychosocial needs
  • Communicate effectively with residents, families, and the care team to support coordinated care
  • Follow care plans, infection control practices, and all safety policies and procedures
  • Document care provided and participate in team meetings as required

Requirements:

  • Active, Home Health Aide certification (required)
  • Proficient in reading, writing, and following instructions in English (required)
  • High school education or GED preferred
  • Demonstrate physical ability to stand, sit, walk, bend, squat, and lift/carry up to 20 pounds unassisted or 50 pounds with assistance
