We are working on behalf of Harigey Council and are looking for a dedicated and compassionate Personal Advisor to join their Leaving Care Service, supporting young people as they transition from care into adulthood. This is a rewarding opportunity to make a real difference to the lives of care-experienced young people by helping them build independence, resilience, and positive futures. You will work with a diverse caseload of care leavers, providing tailored support across housing, education, employment, health, and wellbeing, while ensuring their voices are central to all planning and decision-making.
Job Responsibility:
Support care leavers to successfully transition to independent adulthood
Develop, implement, and regularly review Pathway Plans in line with assessed needs
Manage an individual caseload, maintaining accurate and timely case records
Coordinate multi-agency support, working closely with families, carers, and partner organisations
Support young people to access and sustain accommodation and claim welfare benefits
Encourage and enable engagement in education, training, and employment
Work with care leavers who may have complex needs, including mental health, disability, parenting, or immigration issues
Promote safeguarding and escalate concerns where there is a risk of harm
Participate in duty rotas, team meetings, supervision, and service development activities
Requirements:
Experience working with care leavers, young people, or vulnerable adults
Strong understanding of leaving care legislation, safeguarding, and statutory guidance
Experience of case management and multi-agency working
Ability to build positive, trusting relationships with young people
Strong organisational, communication, and record-keeping skills
Confidence using IT systems and electronic case files
Willingness to work flexibly, including occasional out-of-hours work and visits
