Job Description:

Hello Neighbor is seeking a Perinatal Case Aide/ Manager to provide culturally responsive, trauma-informed perinatal case management services to pregnant refugees and immigrants enrolled in the Smart Start program. The Perinatal Caseworker supports clients throughout pregnancy and into the postpartum period by providing perinatal education, healthcare navigation, advocacy, and connection to community-based resources. This role focuses on promoting healthy pregnancies, positive birth experiences, and early postpartum stability through individualized, client-centered service planning. As part of the Family Health Services Department, the Perinatal Caseworker works closely with clients to address cultural, linguistic, and systemic barriers to care, empower informed decision-making, and support access to prenatal, birthing, and postpartum services. This position collaborates with internal Hello Neighbor programs and external partners while maintaining a clear focus on perinatal support rather than intensive or stabilization case management.

Job Responsibility:

  • Maintain an active caseload of pregnant clients enrolled in the Smart Start program, including referral management, intakes, assessments, and implementation of individualized perinatal service plans
  • Provide culturally responsive, trauma-informed perinatal case management throughout pregnancy and into the postpartum period
  • Support clients with prenatal, birthing, and postpartum healthcare navigation, including scheduling appointments and coordinating follow-up care
  • Provide perinatal education, including preparation for birthing in the U.S., newborn care education, and postpartum planning
  • Support the development of birth plans in collaboration with clients and healthcare providers, as appropriate
  • Connect clients to community-based resources related to perinatal health, basic needs, and family support
  • Provide extended cultural orientation related to healthcare systems, transportation, and postpartum resources
  • Assist clients with transportation planning, including bus training and referrals to the Medical Assistance Transportation Program (MATP)
  • Maintain accurate, timely, and complete client case files in accordance with program and quality assurance requirements
  • Collaborate with Hello Neighbor programs and external partners to ensure coordinated, non-duplicative service delivery
  • Participate in regular supervision meetings, staff meetings, and trainings
  • Uphold confidentiality guidelines and ensure appropriate handling of sensitive information
  • Other duties as assigned within the scope of the position

Requirements:

  • Bachelor’s degree in social work, public health, human services, or a related field
  • Experience working with pregnant individuals, new parents, refugees, immigrants, or other vulnerable populations
  • Knowledge of perinatal health, prenatal and postpartum care systems, or maternal–child health preferred
  • Bilingual or multilingual skills preferred, particularly in priority languages (e.g., Dari, Pashto, Arabic, Swahili, French, Spanish, Haitian-Creole, Rohingya)
  • Demonstrated ability to provide trauma-informed, culturally responsive support
  • Strong interpersonal, communication, and organizational skills
  • Ability to manage multiple clients and tasks while meeting documentation and program requirements
  • Digital literacy and ability to learn new software systems
  • Valid driver’s license, access to a personal vehicle, and proof of insurance
  • Personal auto insurance with a minimum $300,000 single combined limit of liability coverage
  • Flexibility to work occasional evenings and weekends as needed
  • Alignment with and support of Hello Neighbor’s mission and core values
  • Clearances required: Pennsylvania Child Abuse History Clearance, Pennsylvania State Police Criminal History Clearance, Federal Bureau of Investigations (FBI) Criminal History Clearance (PA Act 23 & Act 24)
  • Mandatory Reporter and/or PSEA training may also be required depending on job requirements
  • Must be currently authorized to work in the United States
