Master’s Degree
Current California Nurse Practitioner License
Current BLS for Healthcare Providers, issued by AHA
Furnishing number, State of California (must obtain within 1 year of employment)
DEA license for position that require the writing of scheduled medications (must obtain within 1 year of employment)
Medicare Billing Number (NPI-National Provider Identifier) - (must obtain within 6 months of employment)
Certification: Nurse Practitioner, National (must obtain within 1 year of employment)
Successfully complete the credentialing process through Medical Staff Service
Successfully complete the requirements of the delegations of Services for the assigned division

Job Responsibility

Provides comprehensive physical assessment, treatment, psychosocial support, referral, and health education to children and families in clinical, school, and in-home settings
Functions as a primary care provider, clinical leader, educator, researcher, care coordinator and consultant and works with the supervision of a physician
Performs within the scope of licensures and in accordance with hospital policies
Some positions will be responsible for being on call according to a predetermined schedule or as needed
