Job Description:

Join our client’s Primary Care team as a PCN Pharmacist and make a real difference to patient care in the local community. This role involves conducting clinical medication reviews, independently prescribing as part of a multidisciplinary team, providing medicines optimisation advice, collaborating with other clinicians to deliver care, and maintaining detailed patient records. The job is based in Herefordshire, with potential for hybrid/remote options in the future based on performance and service needs.

Job Responsibility:

  • Carry out clinical medication reviews for patients with long-term conditions
  • Prescribe independently as part of the multidisciplinary team
  • Provide expert advice on medicines optimisation
  • Collaborate with GPs, nurses and other clinicians to deliver high-quality care
  • Keep accurate and detailed patient records

Requirements:

  • GPhC registration as a Pharmacist
  • Independent Prescriber qualification (essential)
  • Minimum of 2 years’ experience in Primary Care settings
  • Excellent communication and clinical decision-making skills
  • A proactive and flexible approach to patient care
