Requirements

GPhC registration as a Pharmacist
Minimum 6–12 months’ experience in a PCN or GP setting
Strong working knowledge of EMIS or SystmOne
Excellent communication, clinical judgment, and patient counselling skills
Independent Prescriber status preferred but not essential
Commitment to full-time availability (Monday to Friday, 9am–5pm)

Job Responsibility

Conduct Structured Medication Reviews (SMRs) and follow-ups
Support long-term condition management (e.g., hypertension, asthma, diabetes)
Manage repeat prescriptions, medicines reconciliation, and deprescribing
Provide medicines-related advice to clinicians and patients
Implement medicines optimisation initiatives and cost-saving audits
Assist with QOF and IIF targets, improving prescribing safety and outcomes
Use EMIS or SystmOne to maintain accurate patient records and documentation
