Overview: Must be able to apply numerous techniques for lifting, moving and transferring patients. Knowledge of transfer methods to decide which transfer method is the safest choice for each patient. Competent in patient lifting, transfer, positioning and transport. Competent in providing patient safety during movement. Knowledgeable in CPR, infection control, customer relations, equipment management and associated policies and procedures. Work with the healthcare team to perform purposeful rounding on the units utilizing pre-defined response criteria.
Job Responsibility:
Patient Transfers: Safely transport patients throughout the facility using appropriate equipment and precautions. Assist with transfers via wheelchair, stretcher, or vehicle, and accompany ambulatory patients as needed. Utilize all available transfer equipment to ensure safe movement
Patient Lifting & Mobility Support: Serve as a knowledgeable resource in the safe use of lifting and transfer equipment. Collaborate with team members to safely lift, reposition, and mobilize patients, ensuring proper body mechanics and adherence to safety protocols
Patient Safety: Consistently follow safety standards, including the use of gait belts, stretcher and bed brakes, and mechanical lifting devices to ensure patient and staff safety at all times
Timely Response & Communication: Remain accessible via phone, walkie-talkie, and care coordination systems. Respond to all transport and assistance requests within 10 minutes. Accurately prioritize and coordinate patient escort requests and departmental needs
Documentation & Reporting: Maintain accurate logs of safe lift and transport activities and submit documentation at the end of each shift. Promptly report any issues or concerns to the charge nurse, nurse supervisor, or manager
Equipment Maintenance: Clean and inspect stretchers, wheelchairs, and other equipment after each use. Apply appropriate indicators (e.g., “clean” tape) and ensure cleaning stations are properly stocked
Patient Care Support: Assist with positioning, turning, ambulation, and movement during patient care activities, including complex procedures and dressing changes. Participate in purposeful hourly rounding alongside the care team
Shift Handoff & Team Collaboration: Provide thorough and accurate handoff communication to incoming team members and work collaboratively with the healthcare team to support patient care goals
Requirements:
Previous patient care experience preferred
Ability to safely lift over 50 pounds and push/pull up to 200 pounds without restrictions
Strong communication and customer service skills required
Current Basic Life Support (BLS/CPR) certification
Successful completion of Lift Team, Transport Team, and Patient Services orientation
Completion of body mechanics training with the Physical Therapy department, with required reassessment of physical competency every 6 months (or as needed)
Ability to meet the physical demands of the role on an ongoing basis, including maintaining functional fitness
Ability to read, write, and perform basic math
Comfortable working in a fast-paced environment and under pressure
Effective communication with patients, staff, and visitors
Ability to perform routine, repetitive tasks following established procedures with close supervision
High school graduate or equivalent
